ABSTRACT

Objective:To explore the cause and treatment of chylothorax after surgery for congenital heart disease(CHD) in newborns.Methods:A retrospective analysis was made to the clinical data of 49 newborns with chylothorax after surgery for CHD within the period from January 2009 to December 2019. These newborns were aged from from 1 day to 28 days with the weight from 2.0 kg to 4.1 kg. The complete transposition of great arteries was performed in 13 cases, coarctation of the aorta/ interruption of the aortic arch in 13 case, right ventricular outflow tract reconstruction/ Blalock-Taussing shunt in 9 cases, total anomalous pulmonary venous connection in 8 cases, ventricular septal defect repair and atrial septal defect repair in 4 cases, ligation of patent ductus arteriosus in 1 case and persisten truncus arteriosus in 1 case. Chylothorax occurred in the right in 19 cases, left side in 20 cases, bilateral in 9 cases and the pericardium in 1 case. The diagnosis was made at the time from 1 day to 22 days after the surgery with an average of 8 days.Results:43 patients were cured(87.75%), 41 cases(83.67%) were cured with diet and support therapy, the course lasted from 4 days to 65 days with an average of 11 days; 1 cases, because of the poor effect of diet and support therapy, was given pleural injection of high-sugar combined with octreotide treatment; 1 case received thoracic duct ligation as the conservative therapy was ineffective; 6 cases of death due to heart failure/ severe pulmonary hypertension after operation, and parents gave up.Conclusion:Individualization conservative therapy is the first choice for chylothorax, while timely surgery can raise the survival rate and save the hospitalization time and the cost.

ABSTRACT

Objective To screen patients with clopidogrel resistance and develop accurate anti-platelet therapy strategies based on CYP2C19 gene polymorphism.Methods A total of 200 patients with acute coronary syndrome/percutaneous coronary angioplasty were selected.The pyrophosphate sequencing technology was using to detect CYP2C19 gene polymorphism for identifying clopidogrel poor metabolizers and guide antiplatelet therapy according to the development of clopidogrel resistance strategies.Results The proportion of patients with clopidogrel resistance (including intermediate metabolizers and slow metabolizers) was about 63.5%.In clopidogrel intermediate metabolizers, 41.4% cases were applied double dosages of clopidogrel and about 20.2% patients were given ticagrelor instead of clopidogrel.For clopidogrel slow metabolizers, approximately 39.3% cases were given doubled dosage of clopidogrel and 46.4% cases were given ticagrelor.Compared with previous data, the proportion of individual treatment of clopidogrel resistance was significantly increased in this study.Conclusion The strategies for the treatment of clopidogrel resistance, which based on the polymorphism of CYP2C19 gene, have been achieved initial success.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effect of ouabain on intracellular Ca(2+) concentration ([Ca(2+)]i) in thoracic aorta vascular smooth muscle cells (VSMCs) in vitro.</p><p><b>METHODS</b>Primary SD rat thoracic aorta VSMCs were cultured by tissue adherent method and identified by immunochemistry. The binding ability between ouabain and VSMCs was detected by autoradiography, and fluo 3-AM (a Ca(2+) fluorescent probe) was employed to investigate whether ouabain affected VSMCs within a short period of time. The effect of a truncated fragment of the sodium pump α2 subunit was assayed in antagonizing the effect of ouabain on [Ca(2+)]i in the VSMCs.</p><p><b>RESULTS</b>Within the concentration range of 0.1-100 nmol/L, ouabain was found to dose-dependently bind to the VSMCs. Different concentrations of ouabain (0-3200 nmol/L) caused a transient, dose-dependent increase in [Ca(2+)]i in the VSMCs, which was antagonized by the application of the truncated fragment of sodium pump α2 subunit.</p><p><b>CONCLUSIONS</b>Elevations in [Ca(2+)]i in the VSMCs can be the cytological basis of high ouabain-induced hypertension. The truncated fragment of the sodium pump α2 subunit can antagonize ouabain-induced increase of [Ca(2+)]i in the VSMCs, which provides a clue for understanding the pathogenesis of and devising a therapeutic strategy for high ouabain-induced hypertension.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To prepare polyclonal antibodies against sodium pump alpha 2 subunit M1-M2 extramembrane fragment (NKAα2 EM1) for studying the pathogenesis of hypertension.</p><p><b>METHODS</b>According to the GenBank data, the amino acid sequence of NKAα2 EM1 was obtained and the target peptide (LAAMEDEPSNDN) was synthesized using a peptide synthesizer with Fmoc method and purified with high-performance liquid chromatography. The synthesized peptide was then coupled to KLH for immunizing New Zealand white rabbits for 4 times to obtain the antiserum. The IgG antibodies against the synthetic peptide, after affinity purification with Protein A, were used for detecting NKAα2 EM1 expression in rat aortic vascular smooth muscle cells by enzyme-linked immunosorbent assay and immunocytochemistry (ICC).</p><p><b>RESULTS</b>The synthesized peptide fragment , which consisted of 13 amino acid residues including one derivatized cysteine residue in the N-terminal (LAAMEDEPSNDN-C), had a theoretical relative molecular mass of 1408.48 D with a measured relative molecular mass of 1407.90 D and a purity exceeding 85.5%. The titer of the antiserum was more than 1:512 000, and the purified IgG antibody concentration was 0.965 mg/ml after purification with Protein A. At a 1:1000 dilution (final concentration of 1 µg/ml), the titer of the purified IgG antibody was more than 1:256 000. The purified IgG antibody could be used at 1:100 to 1:200 dilutions for for immunocytological examination of formalin-fixed cells.</p><p><b>CONCLUSION</b>The anti-NKAα2 EM1 polyclonal antibodies obtained can be used in ELISA and immunocytochemistry for detecting the sodium pump alpha 2 subunit in formalin-fixed tissue or cells to facilitate investigation of the relationship between sodium pump and hypertension.</p>

ABSTRACT

Objective In order to screen effective traditional Chinese medicine compounds to prevent and control porcine respiratory disease syndrome ( PRDC) , seven compound preparations of traditional Chinese medicine were tested and to analyze their antitussive and expectorant effects in mice .Methods Two hundred 6-week old ICR mice ( male∶fe-male=1∶1) were used in this study .Dextromethorphan and ammonium chloride were used as positive control drugs , and physiological saline was used as blank control .The antitussive and expectorant effects of the seven Chinese medicine com-pounds (groups 1, 2, 3, 4, 5, 6, 7) were observed by ammonia-induced cough model and tracheal phenol red secretion method in mice .Results The results showed that compounds 7 and 5 significantly prolonged the cough incubation period (P <0.05), and reduced the cough times within 5 min (P <0.05).Except for the group 4, tracheal phenol red excre-tion in the other groups was significantly lower than that of blank control group (P <0.05), and phenol red excretion in the mice of groups 7, 5 and ammonium chloride group was significantly lower than that in other treatment groups ( P <0.05).Conclusions The Chinese medicine compounds 5 and 7 show most evident expectorant effects , and worthy of fur-ther validation of them as a drug in the treatment of porcine respiratory disease syndrome .

ABSTRACT

Ticks transmit various diseases to livestock and human beings. The researches on ticks are of great importance in medical and veterinary sciences. To express a glutathione peroxidase gene (HlGPx) from Hemaphysalis longicornis in Escherichia coli (E. coli) so as to provide basis for further studies, the gene was amplified from cDNAs of H. longicornis adult ticks by PCR prompted by information from EST library. The TGA codon encoding selenocysteine in the gene was mutated into the universal genetic code for cysteine,TGC, by site-directed mutagenesis. E. coli was transformed with the recombinant expression vector pGEX-4T-1/HlGPx'(the mutated HlGPx) and induced to express recombinant HlGPx', which was then analyzed by sodium dodecylsulphate-polyacrylamide gel electrophoresis and Western blotting. The result showed that the complete coding sequence of HlGPx was obtained successfully, the deduced polypeptide was 223 aa, with a calculated molecular mass of 25.0 kDa; the recombinant fusion protein was approximately 51 kDa, correspondent to the calculated. The antibody against glutathione S-transferase (GST) recognized the recombinant protein fused with GST in a Western blotting assay, which confirmed the result above-mentioned. In conclusion, the mutated HlGPx was expressed in E. coli successfully and it could be used for further preparation of immune serum and functional analysis.

ABSTRACT

ObjectiveTo compare the efficacy and tolerability of 1-week 1% terbinafine hydrochloride cream, 1- and 4-week 2% miconazole nitrate cream in the treatment of interdigital tinea pedis, and to observe the relapse in patients treated with these regimens. MethodsA multi-center, randomized, double-blind and parallel group study was conducted. By using a stratified randomization protocol, patients were divided into 3 groups to apply terbinafine cream twice daily for 1 week and inert cream(placebo) for the next 3 weeks (1week terbinafine group), miconazole cream twice daily for 1 week and inert cream(placebo) for the next 3 weeks (1-week miconazole group), and miconazole cream twice daily for 4 weeks (4-week miconazole group),respectively. Clinical and mycological assessment was made on week 1, 3, 4, 6, 9 and 12 after the initiation of treatment. ResultsA total of 152 patients with positive baseline mycological culture were eligible for the efficacy analysis. After 4-week treatment, the mycological cure rates were 94.7%, 87.8% and 82.6%, global effective rates 89.5%, 81.6% and 63.0%, respectively for the 1-week terbinafine group, 4-week miconazole group and 1-week miconazole group. On week 12, the mycological relapse rates in 1-week terbinafine, 4-week miconazole and 1-week miconazole group were 13%, 14% and 21% respectively, and the incidence of adverse reaction was 2.38%, 2.38% and 3.57%, respectively. ConclusionsAs far as the efficacy and recurrence in patients are concerned, the 1-week terbinafine cream regimen is similar to the 4-week miconazole cream regimen for the treatment of interdigital tinea pedis.

ABSTRACT

<p><b>OBJECTIVE</b>To study the distribution and dynamic change of coumarins in the leaf and root of Dendrobium thyrsiflorum at different developmental stages, and provide an experimental evidence for utilizing the medicinal plant.</p><p><b>METHOD</b>The leaf and root materials were collected at vegetative and reproductive stage. By applying laser scanning confocal microscopy, relative quantity of coumarins of each sample was observed and analyzed.</p><p><b>RESULT</b>Coumarins located in D. thyrsiflorum leaf except fibers in vascular bundles which were close to the upper or lower epidermis. Coumarins were found in D. thyrsiflorum root structure at different developmental stages and walls of cells within cortex were especial appropriate places for the components distribution. Relative quantity of coumarins was changed obviously when plant was growing. The components reached the highest level during profuse flowering period whereas there were less coumarins at vegetative period and little during initial fruit period.</p><p><b>CONCLUSION</b>In order to utilize D. thyrsiflorum reasonably and thoroughly, coumarins should be extracted from the plant leaf and root.</p>

ABSTRACT

OBJECTIVE To study the inhibitory effects of the water extractions from ordinary green tea,black tea,chrysanthemum tea,oolong tea and Puer tea on digestive tract bacteria.METHODS Incubating plate with foveolae was used to study the inhibitory effect on Vibrio parahaemolyticus,Escherichia coli,Salmonella and Shigella with the water extractions of the tea.RESULTS It is showed inhibitory effect on V.parahaemolyticus by the first water extraction from ordinary green tea,black tea,chvysanthemum tea,oolong tea and Puer tea.Same effect was also observed to E.coli,Salmonella and Shigella.The green tea and Puer tea showed best inhibitory effects on V.parahaemolyticus,and the inhibitory zone diameter of the first water extraction of 2.5mg/ml teas were 10.6mm and 12.3mm,respectively.CONCLUSIONS The first water extraction of many kinds of the tea showes inhibitory effects on the digestive bacteria.

Objective To study the action of Mycoplasma genitulium (Mg) in pathogenesis of chronic non-bacterial prostatitis. Methods Mg culture and polymerase chain reaction (PCR) were perfomed on prostatic secretion specimens from 987 patients with chronic non-bacterial pristatitis and 125 healthy men. Results ①The positive rate of Mg in the prostatic secretion from the 987 patients was 31.5% (302/987). Aim To investigate the protection effect of icariin on isoproterenol induced neonatal rat cardiomyocytes injury.Methods Isolation of cardiomyocytes from Sprague-Dawley rats aging 1~3 d were performed.Cardiomyocytes were grown in DMEM culture medium containing 15% fetal bovine serum.The cell density was adjusted to 1?105~5?105 cells?L-1,and the suspension was pipetted into each well of culture plates.Cells were incubated at 37 ℃ in 5% CO_2 atmosphere for 72 h.Cultured rat cardiomyocytes were divided into different groups:control,0.5 mmol?L-1 isoproterenol(model),and therapeutic groups with 0.1,1 and 10 ?mol?L-1 icariin.Cell viability was analyzed using MTT assay.The potential of mitochondrial membrane was determined by laser scanning confocal microscope.Apoptotic cells were detected by using flow cytometric analysis.Results Compared with the control group,the cell activity was lower in model group(P

ABSTRACT

Objective To study the characteristics of articles, citations and authors in Chinese Journal of Neurology(Chin J Neurol). Methods The date about articles, citations and authors of Chin J Neurol from 2002 to 2004 were collected and analyzed in terms of bibliomertic parameters. Results There were 648 papers published in Chin J Neurol during 3 years and papers with funded projects accounted for 25.0%. The average number of articles per isseu was 36; the average number of article per page was 0.43; the average publishing cycle of the articles was 10 months. The authors distributed in 27 mainland provinces and Hongkong, 91.5% of whom shared with co-author; the coporation rate was 4.19. The papers with citation accounted for 65.3% and 95.0% of all the citations came from journals. The Price’s indicator was 56.6%. Conclusions Chinese Journal of Neurology, an academic journal of high quality, with strong influence, pushes the subject of neurology in China forward.

ABSTRACT

Objective To study the effects on berberine hydrochloride on the mitochondria membrane potential and free intracellular calcium of HaCaT cells, and elucidate the mechanism of action of berberine on keratinocytes. Methods Rhodamine-123 fluorescence (very sensitive to mitochondria membrane potential) and Fluo-3/AM fluorescence (suitabe to detect free intracellular calcium in single HaCaT cell) were measured by laser scanning confocal technique. Results Fluo-3/AM fluorescence intensity of HaCaT cells was persistently increased after treating with berberine at concentrations of 5 ? 10-5M, 2.5 ? 10-5M and 1.25 ? 10-5M, and significant differences were observed as compared with the PBS control. The intensity of rhodamine-123 fluorescence in HaCaT cells was decreased immediately when exposed to berberine, with significant difference from that of the PBS control. Conclusions It is suggested that berberine could increase free intracellular calcium and decrease mitochondria membrane potential of HaCaT cells, induce overload of intracellular calcium, influence energy metabolism, and then inhibit the proliferation of keratinocytes.

ABSTRACT

AIM: To explore the molecular mechanism of Radix Ophioponis against vascular endothelial cell (VEC) apoptosis induced by LPS. METHODS: The apoptosis of human umbilical vascular endothelial cells(HUVEC) was induced by lipopolysaccharide(LPS). The influence of Radix Ophiopogonis on the expression of bcl-2 and intracellular Ca 2+ was detected by flow cytometry and laser confocal microscope. RESULTS: The serum containing Radix Ophiopogonis suppressed the increase in bcl-2 expression and overloading of Ca 2+ induced by LPS in HUVEC.CONCLUSION: The mechanism of Radix Ophiopogonis against HUVEC apotosis may be related with its regulatory effect on bcl-2 expression and remission of Ca 2+ overloading.
